29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry: (a) On or about 09/27/2019 - Fuel House (a.k.a. Teepee), employees entering the silo were exposed to an engulfment hazard due to the accumulation of sawdust and adequate measures were not implemented to ensure timely rescue when emergency situations occurred.
