29 CFR 1926.1204(d)(2): The employer did not ensure that proper ventilation equipment was provided and used to obtain acceptable entry conditions: a) On or about 9/20/2019, 304 Front Street, Binghamton, NY, Tank #1: An employee was required to enter a 10,000 gallon gasoline storage tank, to clean and prepare it for removal. The employer did not provide and ensure the use of ventilating equipment such as, but not limited to, forced air ventilation to eliminate the hazard of a flammable atmosphere, resulting in a deflagration and fire when an electric drill was used inside the permit required confined space. Abatement verification is required for this item.
