29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about September 12, 2019, the employer failed to ensure there was a lockout device on the 480 volt disconnect panel box for the chiller being worked on. Employee(s) were changing and expansion valve on the chiller. Employee(s) were exposed to the unexpected start-up of the machine.
