29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Pentair Water Filtration Company located at 220 Park Dr., Chardon, OH 44024: On or about August 19, 2019, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d) and required for the control of the hazardous energy sources such as, but not limited to, gravity were followed before employees conducted activities such as, but not limited to, removing jammed plastic material from the rotor blades/knives of the company's (RotoGran) granulating machinery. Employees were exposed to amputation hazards.
