(A) All belts, pulleys, gears, shafts, and moving parts were not guarded as required. (a) On or about August 22, 2019, in the Work Shop Area, employees utilized a Rigid Table Saw (Model#: TS24241, Serial#: 99300P0145) in the wood cutting and prop making process, without a guard covering the belt and pulley drive shaft of the motor.
