On or about August 08, 2019, and at times prior thereto: a. In the furnace area where employees were exposed to electrical, struck-by and heat hazards, when performing service or maintenance on D4 furnace without utilizing an effective energy control procedure. b. In the furnace area where employees were exposed to electrical, struck-by and heat hazards, when performing service or maintenance on D5 furnace without utilizing an effective energy control procedure. c. In the furnace area where employees were exposed to electrical, struck-by and heat hazards, when performing service or maintenance on D6 furnace without utilizing an effective energy control procedure. d.
