29 CFR 1910.147(f)(2)(i): Whenever outside personnel were engaged in activities covered by the scope and application of this standard, the on-site employer and the outside employer did not inform each other of their respective lockout or tagout procedures: The employer is failing to protect employees from struck-by and crushing hazards. This was most recently documented on or about July 31, 2019 at the Youngers & Sons Manufacturing Company, Inc. facility located at 19223 K42 Highway Viola, Kansas. Employee engaged in orientation of the spindle activities at the Southeast corner of the shop area by the Cincinnati Milacron Horizontal Machining Center (Mill machine, Work Center #1036), Model Magnum 1000; Serial number 3145A0197-0009; 460 V 3 phase, 60 Hz. was exposed to crushing hazard. An employee of Fives Machining Systems Incorporated, contracted to perform servicing and maintenance on the Cincinnati Milacron Horizontal Milling Machine that included multiple energy sources, was exposed to struck-by and crushing hazards in that the host employer and the contracted employer had not informed each other of their respective lockout tagout procedures.
