29 CFR 1910.132(a): Protective equipment was not used when necessary when a hazard of process or environment capable of causing injury or impairment of the body through physical contact was encountered: a) Ethanol Plant, Ethanol Pump/Loading Area - On or about July 24, 2019, the employer did not require employees to wear protective outer garments such as flame-resistant clothing when handling flammable liquids, thus exposing employees to potential thermal burns. b) Ethanol Plant, Ethanol Pump/Loading Area - On or about July 24, 2019, employees were not protected from falls when standing and/or walking on top of a fuel tanker truck during the process of adding gasoline denaturant to the truck, thus exposing employees to fall hazards up to approximately 9 feet.
