29 CFR 1910.421(d)(2): Assessments that were made during planning of dive operations did not include all surface and underwater hazards: a) Intake Drop Pipe - On June 21, 2019, and at times prior, an assessment of the dive operation did not include verification that residual air pressure previously applied to the system was relieved to control the hazards created from air pressure causing an impact from released air and/or debris from a 12 inch drain pipe clog.
