29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design: (a) 1121 N Jefferson St, Jackson MS - On or about May 20, 2019 the employer allowed employees to work from a wood pole scaffold which were missing mid-rail(s), was not fully planked and was approximately 18 inches away from the face of work exposing them to a fall hazard of approximately 20 feet to the ground.
