29 CFR 1910.119(e)(5): The employer did not complete actions as soon as possible with regard to process hazard analysis recommendations that it had resolved to address/complete: a) Ammonia refrigeration system: In 2016 the Process Hazard Analysis (PHA) recommended the installation of a second emergency exit from the roof; as part of their system to promptly address their PHA team's findings and recommendations, the employer resolved the recommendation by accepting to install the second emergency exit, however, the employer failed to complete or begin the action of installation of the second exit, and had no plan to do so, on or about 4/10/19 and 4/16/19. This hazardous condition exposed employees to the hazard of toxic ammonia inhalation and burns. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
