29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. Warehouse Area. On or about March 30, 2019 through April 8, 2019, the employer did not ensure that each powered industrial truck operator had successfully completed training consisting of a combination of formal instruction through computer based training, videos, written exercises, etc. and an evaluation of operating performance (driving test) in the workplace at least once every three years. Employees were exposed to struck-by injuries. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
