29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts or flying chips: a) De-barker machine: On April 4, 2019 and at times prior thereto, a complete guard was not in place on the tail spool of waste chain conveyor, exposing employees to an in-running nip point. b) Chipper out-feed: On April 4, 2019 and at times prior thereto, a guard was not in place on the tail spool of the chipper out-feed, exposing employees to rotating parts. c) Blower Shaft: On April 4, 2019 and at times prior thereto, a guard was not in place around a rotating shaft, exposing employees to the rotating part. d) Re-chipper: On April 4, 2019 and at times prior thereto, a guard was not in place on a rotating shaft, exposing employees to rotating parts.
