29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. a) House driveway: Employees used an electric powered demolition hammer to break concrete pavement. The demolition hammer was connected to an extension cord, which was routed to an outlet device in the basement. The extension cord was missing a ground pin. On or about 04/02/19.
