29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. At the residential home located at 1110 Harbor Haven, Southlake, Texas, the employer did not instruct employees in the recognition and avoidance of unsafe conditions that could exist from using MAYCO and Western plaster pumps and their components including but not limited to hoses, mixers, couplings, and nozzles, exposing employees to struck-by hazards.
