29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) On or about 03/12/2019 in the CSF room; where the interlocked door (a small opening in the machine guard) did not protect the operator from point of hazards. The rotational momentum of the machine (tablet press MF-13) did not stop when the interlocked door was opened, allowing the operator to access the point of operation and other pinch points before the machine came to a complete stop.
