29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) Throughout Phase 1 Building (Matline) and Phase 2 Building - On or about March 7, 2019 and times prior to, employees who performed maintenance and servicing activities on equipment did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy associated with each type of equipment, and how to isolate and control the different energy sources, thus exposing employees to caught-in, struck-by, and crushed-by hazards.
