29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: Employees were exposed to crushing hazards. On or about February 20, 2019 a contract / temporary worker engaged in welding a crack in a metal hopper for the North Carolina Line was exposed to a crushing hazard. No lockout / tagout was used to control all energy sources. Canyon Stone and the contract / temporary worker did not communicate to each other their respective lockout / tagout procedures prior to starting the welding work on the North Carolina line.
