29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. a)On 2/27/2019 the employer did not provide devices such as but not limited to wedges to secure energy sources such as kinetic energy sources for the Marquip Corrugator. An employee was utilizing a grinder with a polisher attachment (Dewalt 4.5 angle grinder Model # D28402) on the rollers of the corrugator, this caused the rollers to free spin, as the employee applied pressure on the grinder the employee's hand slipped, causing the hand to go in between two of the rollers.
