29 CFR 1910.119(l)(3): Employees involved in operating a process and maintenance and contract employees whose job tasks would be affected by a change in the process were not informed of and trained in the change prior to start-up of the process or affected part of the process: On and before February 10, 2019, when startup activities began after a turnaround, no training on the DU2 Startup from Cold Circulation Procedure or the Establishing Cold Circulation from Unit Startup Procedure had been given to affected employees, including those who were involved in the startup.
