29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: On or about February 7, 2019, and at times prior to, at a bridge sandblasting/painting project located on highway 69, 4 miles south of Muskogee, OK, an employee was not instructed in the recognition and avoidance of unsafe conditions when working on a scaffold, exposing the employee to fall hazards.
