29 CFR 1910.184(c)(9): Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling(s): (a) Metro Machinery Rigging Inc. at 20670 County Road 85 Tower 55 Fleming, CO 80728: On or before February 5, 2019 the exposing employer did not ensure that employees were kept clear of loads being lifted by slings. A JLG G15-44A Telehandler forklift was being used along with chains connected to the forks to lift counterweights which weighed 22,000 lbs each. An employee was standing in between two stacks of counterweights, approximately 21 inches apart, while one of the counterweights was being hoisted. As the counterweight was hoisted by the slings, the counterweight rotated in the slings and crushed an employee's head in between the two stacks.
