29 CFR 1926.352(e): Additional personnel was not assigned to guard against fire while the actual welding, cutting, or heating operation was being performed; (a) On January 5, 2019, at the construction site at 12000 Lawndale St, Houston TX 77017, the employer did not ensure that additional fire prevention precautions such as the use of a fire watch, was provided when grinding and welding was done on a 16" crude oil pipeline. This condition exposed employees to struck-by and flash fire hazards.
