29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): East Side, adjacent to the roadway: On or about December 14, 2018, employees were exposed to cave-in hazards while connecting pipes without having an adequate trench shield system in place throughout the excavation per the design of the registered professional engineer.
