29 CFR 1910.1200(h)(3)(ii): Employee training did not include the physical and health hazards of the chemicals in the work area: (a) Throughout the facility -- On or prior to 3/21/19, employees working in areas of the plant which contained combustible dust accumulations were not trained in the fire and explosion hazards associated with combustible dusts, including but not limited to use of housekeeping procedures that minimized generation of dust clouds.
