29 CFR 1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs with at least 3 treads and at least 4 risers was equipped with stair rail systems and handrails as specified in Table D-2. The employer is failing to protect employees from slips, trips, and falls when exposed to an enclosed 47 inch wide stairway leading to the basement. The stairway is only equipped with one hand rail. This was most recently documented on or about December 11, 2018 at CSL Woodbridge, LLC located at 1913 East Highway 34 in Plattsmouth, Nebraska. The basement is used for mainly for storage; however, maintenance personnel frequently work in the basement.
