29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b) Location: Worksite located at 1086 Canterbury Street Roslindale MA. Accident: Instance 1) On or about December 2, 2018 an employee sustained fatal injuries from a 26 foot fall to the ground below while working where adequate fall protection had not been used or provided. Instance 2) On or about December 2, 2018 an employee sustained fatal injuries after falling 26 feet to the ground below, while working where a guard rail had not constructed to sustain a 200 pound force without failure
