Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On or before November 29, 2018, the employer failed to ensure that employees who operated the Comco KB-70 bender (model: KB-70NS; S/N: K-02707-N) and performed tooling changes including changing the pressure die, clamp die, bend die, and collet pads during set up had the knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy; perform an orderly shutdown by completely shutting down the bender; isolate the bender from the energy sources; affix lockout or tagout devices to each energy isolating device; relieve, disconnect, restrain, and otherwise render safe all potentially hazardous stored or residual energy for the bender; and verify that isolation and deenergization of the bender had been accomplished. b. On or before December 19, 2018, the employer failed to ensure that employees who operated the Chiyoda SP-90 bender (model: SP-90S; S/N: 8507) and performed tooling changes including changing the pressure die, clamp die, bend die, and collet pads during set up had the knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy; perform an orderly shutdown by completely shutting down the bender; isolate the bender from the energy sources; affix lockout or tagout devices to each energy isolating device; relieve, disconnect, restrain, and otherwise render safe all potentially hazardous stored or residual energy for the bender; and verify that isolation and deenergization of the bender had been accomplished.
