29 CFR 1910.178(m)(3): Unauthorized personnel were permitted to ride on powered industrial trucks. On October 30th 2018 in the B&H Freight Line Inc. warehouse located at 1700 Anaconda Road, Harrisonville Missouri 64701, An employee was exposed to an approximate 10 foot fall hazard. The employee was seen in plain view, standing on and working from a wooden pallet which was lifted to elevation by another employee who was operating a fork truck.
