29 CFR 1910.29(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high. On or about 10/29/2018, at the Traffic Barricade Type H (TBH) form equipment located outside of the facility at 220 Cargill Road Connector, Macon, GA: Employees walking and working on the fabricated metal platform connected to the TBH form equipment did not have a midrail installed on the back side of the platform. The employer failed to ensure a midrail guardrail system was installed.
