29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. At the establishment, in the baler department, employees operated a # 504 Hogger machine that was not equipped with guards around the ingoing nip points of the feed rollers, exposing employees to crushed-by and amputation hazards:
