Bay Shore, NY: Employees were doing foundation wall work from a trench excavation. A 6 ft. Werner stepladder was used for egress in the folded position; on or about 10/19/18. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
