29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: The employer is failing to protect employees from struck-by hazards by not ensuring equipment is not kept at least 2 feet from the excavation edge. This was most recently documented at 150th St, and Capehart Rd., Springfield, Nebraska on or about October 18, 2018. The excavator was on the edge of the excavation increasing the potential of possible cave-in due to the weight of the equipment and the vibration of the equipment while it is in operation.
