Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.178(l)(3)(ii)(I): Powered industrial truck operators did not receive training on unique or potentially hazardous environmental conditions in the workplace that could affect safe operations: a) Warehouse - The employer failed to train employees on crushing and pinning injuries associated with under-ride hazards while operating Crown PC4500 pallet jacks next to horizontal rack beams in the new 3M warehouse. The configuration of the horizontal rack crossbars could enter the open operators compartment, on or about October 11, 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
