Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Employees who performed servicing and maintenance at the 3133 East Kemper Road, Cincinnati, OH, worksite, such as but not limited to the Vulcan parts washer were not required to: (1) know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy, (2) turn off and shut down the machines, (3) physically locate and operate the energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, hot water in the tank, (4) affix lockout or tagout devices to each energy isolating device, (5) prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished. Employees required to rely upon electronic control components were exposed to burn hazards from heat and hot water in the water tank of the Vulcan parts washer. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
