29 CFR 1910.119(e)(1): The employer did not perform an initial process hazard analysis (hazard evaluation) on processes covered by 29 CFR 1910.119: a) Loading Rack, 436 Doremus Avenue, Newark, NJ Employees monitor and operate a covered process which includes 272,000 lbs. of pentane, a flammable liquefied petroleum gas. The initial PHA conducted by the employer did not include the entire covered pentane blending process, specifically after the pentane injection points into the three gasoline products through the loading rack.
