OSH ACT of 1970 Section (5)(a)(1): Mayhew Basque Plastics, LLC, 100 Simplex Dr., Westminster, Massachusetts: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing hazards of falling loads. Overhead gantry crane at 100 Simplex Drive Westminster, MA: On or before September 5, 2018, employees were exposed to struck by and crushing hazards of falling loads while using a rolling gantry crane which had a missing latch on the lifting hook and the safety cable was not attached. The crane is used several times a week to lift molds that weigh in excess of 200 pounds.
