29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before use: a) 626 Lynnway, Lynn, MA On or about August 30, and September 13, 2018, failures in the vapor barrier of insulated piping were not repaired. Piping located in the engine room 2 was found to have frost permeating the pipe insulation. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
