29 CFR 1910.147(c)(7)(i) (A): Each authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) On or about August 23, 2018 and at times prior to, employees employees removed jams from the chain and sprocket, removed contaminated product from the roller and deep clean various equipment including, but not limited to the Tenter #1. Employees were not able to identify and properly lockout all energy sources and were exposed to struck by, burns, caught in and amputation hazards.
