29 CFR 1926.451(g)(3)(i): Verical lifelines were not fastened to a fixed safe point of anchorage, independent of the scaffold, and protected from sharp edges and abrasion. Safe points of anchorage include structural members of buildings, but do not include standpipes, vents, other piping systems, electrical conduit, outrigger beams, or counterweights. a) Jobsite: 3525 I Street, Philadelphia, PA 19134 building Rooftop - Employees conducting masonry work from a two point suspended scaffold on a 5th story window were exposed to fall hazards. A vertical life line used by an employee was tied around an HVAC vent instead of a safe point of anchorage. Vertical life lines were also not protected from sharp edges where they were draped over the building's parapet wall. This was documented to occur on August 8, 2018.
