29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: The employer is failing to protect employees from amputation and crush-by hazards. This was most recently documented at 1351 S. Reca Ct, Wichita, Kansas, where employees were removing and replacing the right and left Boom Hoist Cylinders on a Lull Highlander Model 1044C-54. The employees were exposed in that additions and modifications that affected the capacity and safe operation of the forklift were added without the manufacturer's written approval. a) On or about 07/27/18, a Nissan forklift (CPH02A20V) was used to facilitate the replacement of the right Boom Hoist Cylinder on the Lull. In order to accomplish this task 8 foot slings were added to the forks using Countyline hitch pins (part#18PCH001TSC). The addition and placement of the slings and pins affected the safe operation of the forklift as the calculated center of gravity and lifting capacity of the forklift was modified. Manufacturer's written approval was not provided prior to the addition of the slings and pins. b) On or about 08/03/18, a Toyota forklift (42-6FGU25) was used to facilitate the replacement of the left Boom Hoist Cylinder on the Lull. In order to accomplish this task 8 foot slings were added to the forks using Countyline hitch pins (part#18PCH001TSC). The addition and placement of the slings and pins affected the safe operation of the forklift as the calculated center of gravity and lifting capacity of the forklift was modified. Manufacturer's written approval was not provided prior to the addition of the slings and pins.
