29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about August 1, 2018, the employer failed to ensure that a periodic inspection was performed of their written lock out tag out program at least annually. The program was written and implemented as part of their employee handbook in 2016. The program has deficiencies such as but not limited not having machine specific procedures in place. Employees are exposed to pneumatic and electrical energy sources and hazards such as but not limited to arc flash and electrical shock.
