29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: The employer is failing to reevaluate employees on how to operate the powered industrial trucks and recognize the struck by or caught under hazards associated with their operation. The most recent occurrence was documented during the inspection conducted on July 30, 2018 on the jobsite located at 10004 NW 112th St, Malcolm, NE 68402. Employees use a forklift daily to load materials into their work trucks. The employer has failed to ensure that each operator of the forklift has been evaluated every three years as required.
