29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Reivicart machine, SN: 1330713 - On or about May 22 2018, and at times prior to, employees engaged in tasks including but, not limited to setting up and removing jams from the machine were exposed to amputation hazards where lockout/tagout procedures were not utilized.
