29 CFR 1910.119(e)(3)(i): The process hazard analysis did not address the hazards of the process: a) Radford Army Ammunition Plant, Building 1626, on or about June 11, 2018 - The employer's 2017 Process Hazard Analysis (PHA) of the Solvent Propellant - Finishing/Recovery of the Building 1626 Air Dry Process did not address hazards of the process such as, but not limited to AFP-001 Propellant's sensitivity to friction, shock, impact, and the use of wooden push-down paddles to remove residual propellant from air dry tanks. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
