29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: On or about April 9, 2018, during the installation of roofing material at a job site located at 5245 S. Prospect St, Ravenna, Ohio 44266; the employer did not ensure employees working on a flat roof were protected from a fall of varying heights of approximately 13.7 to 22 feet by utilizing a fall protection system, fall protection training, fall hazard recognition, and ensuring employees understood and comprehended the training.
