29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): At the time of the inspection, it was determined that at the time of the accident on 4/24/2018 the authorized employee did not have adequate knowledge of the methods to control the hazardous energy and the machine was not shut down using procedures established for the machine. The energy isolating devices needed to control the energy to the machine were not operated exposing the employee to a hazardous energy source. Emergency stop buttons are not an approved method to isolate the electrical power supply to the cable slitter. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
