29 CFR 1910.303(g)(2): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures: a) Employees were exposed to electrical hazards, in that an electrical box (480 volts) in the motor control room was not guarded or equipped with a cover or door.
