29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that was less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. a) On or about 04/05/18 at the Lackawanna site, employees were exposed to a fall hazard when the hole, ( 30 inches x 3 inches ) between the junction box and the support rods on the south side of the large blue motor at the top of the work platform south of the crusher, was open. b) On or about 04/05/18 at the Lackawanna site, employees were exposed to a fall hazard when the hole, ( 34 inches x 6 inches ) between the north side of the large blue motor at the top of the work platform and the access ladder south of the crusher, was open.
