29 CFR 1910.23(c)(11): The employer did not ensure that portable ladders used to gain access to an upper landing surface had side rails that extended at least 3 feet above the upper landing surface: a) Atlantic Precast Concrete - On or about March 15, 2018, an employee was using a portable 10 foot ladder to access the top of a concrete form that was 9 foot, four inches (9'4") above grade, and the employer did not ensure that the side rails of the ladder extended at least 3 feet above the working edge. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
